Acyl-CoA synthetase medium-chain family member 3 (ACSM3) is a mitochondrial enzyme that catalyzes the conjugation of coenzyme A (CoA) to medium-chain fatty acids (C4–C14), activating them for entry into downstream lipid metabolic pathways, mainly fatty acid β-oxidation. ACSM3 is highly expressed in the liver and is essential for maintaining mitochondrial energy homeostasis, lipid oxidation, and glucose metabolism. Reduced ACSM3 expression is linked to metabolic syndrome, obesity, hepatic dysfunction, and increased cardiovascular risk. Genetic and expression studies highlight its role in metabolic and inflammatory diseases, and modulation of the p38 MAPK pathway represents a therapeutic approach for ACSM3-associated metabolic disturbance.
Drugs such as adezmapimod indirectly target ACSM3 pathway by inhibiting downstream p38 MAPK signaling, which is activated upon ACSM3 deficiency
