Overview

Adipose tissue health describes the functional integrity and metabolic homeostasis of fat depots, which serve as both energy reservoirs and active endocrine organs. In a healthy state, adipose tissue prevents systemic lipotoxicity by efficiently sequestering excess lipids and secreting insulin-sensitizing adipokines like adiponectin. Conversely, adipose tissue dysfunction is characterized by adipocyte hypertrophy, chronic low-grade inflammation, hypoxia, and fibrosis, which collectively drive metabolic syndrome and type 2 diabetes. Therapeutic strategies to improve adipose tissue health include the use of PPAR-gamma agonists to promote healthy adipogenesis and GLP-1/GIP receptor agonists to mitigate local inflammation and improve systemic insulin sensitivity. Maintaining healthy adipose function is essential for preventing the progression of non-alcoholic fatty liver disease (NAFLD) and cardiovascular complications by ensuring lipids are stored in physiological depots rather than ectopic sites like the liver and muscle.

Mechanism of action

Drugs targeting this physiological state primarily work through Peroxisome proliferator-activated receptor gamma (PPARγ) agonism to promote adipocyte differentiation and lipid sequestration, Glucagon-like peptide 1 (GLP-1) and Glucose-dependent insulinotropic polypeptide (GIP) receptor agonism to reduce adipose inflammation, and the induction of thermogenesis via Beta-3 adrenergic receptors or FGF21 signaling.
