Overview

The adrenal cortex hormone synthesis pathway is a complex biochemical cascade that converts cholesterol into essential steroid hormones, including glucocorticoids, mineralocorticoids, and adrenal androgens [1]. This pathway is compartmentalized within the three layers of the adrenal cortex: the zona glomerulosa, zona fasciculata, and zona reticularis, each possessing a specific set of enzymes that determine the final hormone product [1, 2]. Key enzymes in this process include members of the cytochrome P450 family, such as CYP11A1 (side-chain cleavage), CYP17A1 (17α-hydroxylase), and CYP11B1 (11β-hydroxylase) [2]. Dysregulation of these enzymatic steps leads to significant clinical conditions, such as Cushing's syndrome (cortisol excess), Conn's syndrome (aldosterone excess), and congenital adrenal hyperplasia (enzyme deficiencies) [4]. Pharmacological agents often target specific enzymes within this pathway to suppress hormone overproduction or to treat hormone-sensitive cancers like prostate cancer [3]. However, therapeutic intervention requires precise dosing and monitoring to prevent the development of life-threatening adrenal insufficiency [3, 4].
