Overview

Adrenocortical steroidogenic cytochrome P450 enzymes are a specialized group of heme-containing monooxygenases located in the adrenal cortex that are responsible for the biosynthesis of all steroid hormones from cholesterol (NIH). This group primarily consists of five enzymes: CYP11A1 (cholesterol side-chain cleavage), CYP17A1 (17-alpha-hydroxylase/17,20-lyase), CYP21A2 (21-hydroxylase), CYP11B1 (11-beta-hydroxylase), and CYP11B2 (aldosterone synthase) (UniProt). These enzymes are distributed between the mitochondria and the endoplasmic reticulum, where they catalyze sequential reactions to produce glucocorticoids, mineralocorticoids, and adrenal androgens (StatPearls). Dysregulation of these enzymes, whether through over-expression or genetic mutations, is central to the pathophysiology of diseases such as Cushing's syndrome, primary hyperaldosteronism, and congenital adrenal hyperplasia (NIH). Therapeutic intervention often involves the use of inhibitors like abiraterone, ketoconazole, and osilodrostat to suppress the production of specific hormones (PubMed). However, the use of these drugs requires careful monitoring for potential safety concerns, including adrenal insufficiency, electrolyte imbalances, and hepatotoxicity (StatPearls). The high degree of structural homology between certain isoforms, such as CYP11B1 and CYP11B2, presents a significant challenge for developing highly selective inhibitors (NIH).
