Airway mucus hydration refers to the regulation of water content within the mucus layer lining the respiratory tract. Proper hydration is essential for maintaining optimal mucus viscosity and elasticity, which are critical for effective mucociliary clearance. The primary mechanism controlling airway surface liquid (ASL) volume is active transepithelial ion transport involving sodium absorption via epithelial sodium channels (ENaC) and chloride secretion through CFTR channels. Aquaporins facilitate rapid water movement. Purinergic signaling via ATP release and P2Y2 receptor activation provides autoregulatory feedback. Dehydration impairs mucociliary clearance, contributing to airway obstruction and increased susceptibility to infection in diseases such as cystic fibrosis, chronic bronchitis, and COPD. Therapeutic strategies aim to restore hydration through osmotic agents or pharmacologic modulation of ion channel function.
