Aldehyde and ketone groups, collectively referred to as carbonyl groups, are reactive electrophilic moieties that form on biomolecules such as proteins, lipids, and DNA, primarily through oxidative stress and glycation processes (Dalle-Donne et al., 2003, PMID: 12554058). These groups are hallmark indicators of "carbonyl stress," a state associated with the accumulation of reactive carbonyl species (RCS) like malondialdehyde and 4-hydroxynonenal (Negre-Salvayre et al., 2008, PMID: 18402538). In pathological conditions, these groups react with amino groups on proteins to form irreversible advanced glycation end-products (AGEs) and advanced lipoxidation end-products (ALEs), leading to protein cross-linking and tissue damage (Thornalley, 2003, PMID: 14654083). Therapeutically, these groups are targeted by carbonyl scavengers, such as hydralazine and aminoguanidine, which covalently bind the reactive species to prevent cellular damage (Burcham et al., 2000, PMID: 11042054). Beyond pathology, aldehyde and ketone groups are utilized in biotechnology as bioorthogonal "handles" for site-specific labeling and the production of antibody-drug conjugates (Sletten & Bertozzi, 2009, PMID: 19455540). Their presence serves as a critical biomarker for oxidative damage in various chronic diseases, including diabetes and neurodegeneration.
