Overview

Circulating allergen-specific Immunoglobulin E (IgE) and Immunoglobulin G (IgG) are pivotal components of the humoral immune response to environmental antigens. IgE is the central mediator of Type I hypersensitivity; upon exposure to a specific allergen, IgE bound to high-affinity receptors (FcεRI) on mast cells and basophils triggers the release of inflammatory mediators like histamine, leading to allergic symptoms [Source: NIH/NIAID]. In contrast, allergen-specific IgG, particularly the IgG4 subclass, acts as a blocking antibody that can intercept allergens before they bind to cell-surface IgE, thereby preventing degranulation [Source: PubMed - PMCID: PMC4918332]. These antibodies are primary targets for therapeutic intervention in allergic diseases. Monoclonal antibodies such as Omalizumab work by sequestering free serum IgE, reducing the density of FcεRI receptors on effector cells [Source: StatPearls]. Mechanism of action

Anti-IgE monoclonal antibodies bind to the Fc region of circulating IgE, preventing its attachment to FcεRI on mast cells and basophils, which reduces mediator release and downregulates receptor expression [Source: StatPearls]. Allergen immunotherapy (AIT) induces peripheral T-cell tolerance and promotes the production of allergen-specific IgG4, which acts as a competitive inhibitor (blocking antibody) against IgE-mediated allergen recognition [Source: PubMed - PMCID: PMC4918332].
