Allergen-specific Immunoglobulin E (IgE) recognizing Dermatophagoides farinae Der f 1 and Der f 2 epitopes is a primary driver of allergic sensitization to the American house dust mite. Der f 1 is a 25 kDa cysteine protease that facilitates allergen penetration by cleaving epithelial tight junctions, while Der f 2 is a 14 kDa protein that acts as a functional mimic of MD-2, enhancing lipopolysaccharide-induced TLR4 signaling (PMID: 24502325). These IgE antibodies, once produced, sensitize mast cells and basophils by binding to the high-affinity FcεRI receptor. Furthermore, allergen-specific immunotherapy (AIT) utilizes extracts containing these epitopes to reprogram the immune system, promoting the development of regulatory T cells and the production of IgG4 blocking antibodies that compete with IgE (PMID: 28434615). Clinical management relies on measuring these specific IgE levels to confirm diagnosis and monitor the efficacy of immunotherapy.
