Alloreactive effector T cells are a subset of T lymphocytes that recognize and respond to alloantigens—antigens from genetically distinct individuals of the same species, most notably those presented by major histocompatibility complex (MHC) molecules during organ or tissue transplantation. These cells play a central role in mediating immune responses against transplanted tissues, leading to graft rejection or graft-versus-host disease (GVHD). Upon activation through direct, indirect, or semi-direct pathways, they proliferate, produce cytokines, and kill target cells expressing alloantigens, driving acute and chronic transplant rejection and GVHD. Their high metabolic activity makes them vulnerable to mitochondrial inhibitors. Monitoring their frequency and function is relevant for predicting transplant outcomes and tailoring immunosuppressive regimens. Memory alloreactivity can arise even without prior exposure due to cross-reactivity with pathogen-specific memory pools.
