The Alpha-3 beta-4 neuronal nicotinic acetylcholine receptor is a pentameric ligand-gated ion channel composed of α3 and β4 subunits, primarily located in the autonomic ganglia and specific brain regions such as the medial habenula and interpeduncular nucleus (NIH, 2020). It serves as a critical mediator of fast excitatory synaptic transmission in the peripheral nervous system, regulating autonomic functions including heart rate and gastrointestinal motility (Wikipedia, 2024). In the central nervous system, this receptor subtype is a key component of the habenulo-interpeduncular pathway, which plays a significant role in modulating reward, aversion, and the withdrawal symptoms associated with nicotine and other drugs of abuse (PubMed, 2012). Genetic variations in the CHRNA3-CHRNA5-CHRNB4 gene cluster on chromosome 15 are strongly associated with nicotine dependence and an increased risk of lung cancer (NIH, 2022). Pharmacologically, the receptor is targeted by agonists like nicotine and varenicline, as well as antagonists like mecamylamine and the investigational compound 18-methoxycoronaridine, which are being studied for their potential to treat substance use disorders (ACS, 2022). However, the widespread expression of this receptor in the autonomic nervous system presents a therapeutic challenge, as non-selective modulation can lead to significant cardiovascular and gastrointestinal side effects (StatPearls, 2023).
