Overview

The *alpha–adrenergic receptor* is a family of membrane-bound G protein-coupled receptors that mediate the physiological actions of catecholamines such as norepinephrine and epinephrine in both the central and peripheral nervous systems. There are two main types—alpha–1 adrenergic receptor and alpha–2 adrenergic receptor—each further subdivided into subtypes such as α1A/B/D and α2A/B/C. These receptors play key roles in regulating vascular tone by causing smooth muscle contraction or relaxation depending on subtype location; they also modulate neurotransmitter release at nerve terminals. Clinically important drugs target these receptors for conditions including hypertension (via antagonism or central inhibition), benign prostatic hyperplasia (via selective antagonism), nasal congestion or hypotension/shock states (via selective activation). The diversity among subtypes allows for tissue-specific drug targeting but also presents challenges regarding selectivity versus side effect profiles. Alterations in their function have been implicated in several diseases including cardiovascular disorders.[1][3][4]

Mechanism of action

Alpha agonists stimulate the alpha receptors to mimic catecholamines like norepinephrine and epinephrine. For α1 receptors: Activation leads to stimulation of phospholipase C via Gq proteins → increased intracellular Ca²⁺ → smooth muscle contraction. For α2 receptors: Activation inhibits adenylate cyclase via Gi proteins → decreased cAMP → reduced neurotransmitter release and sympathetic outflow; also causes some smooth muscle contraction.[4][6] Antagonists block these effects—alpha blockers inhibit vasoconstriction leading to vasodilation and lower blood pressure; centrally acting alpha blockers reduce sympathetic tone.

Safety considerations

Alpha agonist side effects can include hypertension (with excessive stimulation), bradycardia (especially with central alpha_2 agonists), sedation/drowsiness (notably with clonidine/dexmedetomidine), dry mouth.Alpha antagonists can cause orthostatic hypotension/fainting due to excessive vasodilation.Central acting drugs may cause rebound hypertension if abruptly discontinued.[8][9]
