Overview

Alternaria alternata allergen-specific Immunoglobulin E (IgE) refers to the subset of antibodies produced by the immune system that specifically recognize and bind to proteins from the mold Alternaria alternata, such as the major allergen Alt a 1 (Gabriel et al., 2016). These antibodies play a central role in the pathogenesis of mold-induced allergic diseases, including allergic rhinitis and severe asthma (Bousquet et al., 2012). Upon exposure to Alternaria spores, these specific IgE molecules, which are typically bound to high-affinity receptors (FcεRI) on mast cells and basophils, cross-link with the allergen, triggering the release of inflammatory mediators like histamine and leukotrienes (Matricardi et al., 2016). This process leads to the clinical symptoms of allergy. Therapeutic strategies targeting this pathway include the use of anti-IgE monoclonal antibodies like Omalizumab, which neutralize free IgE regardless of specificity, and allergen-specific immunotherapy (AIT), which aims to induce immune tolerance to Alternaria allergens (FDA Label for Xolair; EAACI Guidelines). Monitoring the levels of these specific antibodies is crucial for diagnosing mold sensitivity and assessing the efficacy of treatment interventions (Matricardi et al., 2016).
