Overview

Amylase-trypsin inhibitors (ATIs) are a group of non-gluten proteins found in wheat, rye, and barley that serve as natural defense mechanisms for plants against pests by inhibiting their digestive enzymes. In humans, ATIs have been identified as significant triggers of the innate immune system, specifically by acting as ligands for the Toll-like receptor 4 (TLR4) complex in the gut. Unlike gluten, which primarily drives the adaptive immune response in celiac disease, ATIs provoke a rapid inflammatory response characterized by the release of cytokines like IL-8 and TNF-alpha from myeloid cells. This inflammatory activity makes ATIs a primary suspect in the pathology of non-celiac wheat sensitivity (NCWS) and a major factor in the exacerbation of other conditions such as irritable bowel syndrome and celiac disease. Current therapeutic approaches focus on dietary exclusion or the development of enzymatic therapies to degrade these proteins before they can reach the intestinal mucosa. Research is also exploring the use of TLR4 antagonists to mitigate the inflammatory cascade triggered by ATI consumption.

Mechanism of action

Dietary amylase-trypsin inhibitors (ATIs) act as potent activators of the innate immune system by binding to and signaling through the Toll-like receptor 4 (TLR4)-MD2 complex in intestinal myeloid cells. This interaction triggers the release of pro-inflammatory cytokines, which exacerbates intestinal inflammation and can worsen pre-existing systemic immune conditions.
