The anal mucosa is the innermost tissue layer lining the anal canal, serving as a critical physiological barrier between the internal environment and the external world. It is characterized by a distinct histological transition at the dentate line, moving from simple columnar epithelium in the upper canal to non-keratinized stratified squamous epithelium in the lower portion (StatPearls, 2023). This tissue performs essential biological functions, including the secretion of mucus for lubrication and the maintenance of an immunological barrier against enteric pathogens (NIH, 2022). While the anal mucosa itself is an anatomical structure rather than a single molecular target, it is the primary site for the administration of topical therapies and the origin of various pathologies such as anal fissures, hemorrhoids, and anal squamous cell carcinoma (Mayo Clinic, 2024). Pharmacological interventions targeting this region often focus on reducing inflammation, alleviating pain, or treating viral infections like the Human Papillomavirus (PubMed, 2021). Understanding the unique histological and immunological properties of the anal mucosa is vital for developing effective localized drug delivery systems and diagnostic tools for colorectal health.

Mechanism of action

Drugs applied to the anal mucosa act through various molecular mechanisms depending on the active ingredient, such as blocking voltage-gated sodium channels to provide local anesthesia, activating glucocorticoid receptors to reduce inflammation, or donating nitric oxide to induce smooth muscle relaxation (StatPearls, 2023; PubMed, 2021).
