Mechanism of action

This combination utilizes a multi-modal approach to anesthesia. Propofol and sevoflurane primarily act as positive allosteric modulators of the GABA-A receptor to induce hypnosis and sedation, while sevoflurane also inhibits NMDA receptors and activates glycine receptors (StatPearls, 2023). Remifentanil acts as a potent, short-acting agonist at the mu-opioid receptor to provide intense analgesia (NCBI, 2023). Together, they produce a synergistic effect that allows for lower doses of each agent, reducing individual toxicity while maintaining a deep anesthetic state (Anesthesia & Analgesia, 2001).
