Overview

The Anaplastic lymphoma kinase (ALK)-derived peptide presented by human leukocyte antigen (HLA) is a specialized immunotherapy target consisting of a short fragment of the ALK protein displayed on the cell surface by MHC Class I molecules. ALK is a receptor tyrosine kinase that is frequently mutated, amplified, or rearranged in various cancers, such as non-small cell lung cancer and neuroblastoma, making it a potent oncogenic driver (UniProt Q9UM73). Intracellular ALK proteins are processed by the proteasome into peptides, which are then loaded onto HLA molecules (most commonly HLA-A*02:01) for recognition by cytotoxic T-lymphocytes (PubMed: 12060603). This complex is a primary target for next-generation immunotherapies, including T-cell receptor (TCR) engineered T-cells and peptide-based vaccines, which aim to bypass the limitations of small-molecule inhibitors (PubMed: 16452186). Unlike kinase inhibitors that target the protein's enzymatic activity, these therapies leverage the immune system to identify and destroy cells based on the physical presence of the ALK-HLA complex. Clinical development focuses on patients with ALK-positive malignancies who have developed resistance to traditional tyrosine kinase inhibitors. However, therapeutic application requires careful patient selection based on both ALK status and specific HLA haplotypes to ensure effective antigen presentation.
