Overview

Ankyrin-1 is a scaffolding/adaptor protein primarily expressed in red blood cells, where it links integral membrane proteins—most notably band 3—to the underlying spectrin-actin cytoskeleton[1][2][3][5][6]. This connection is essential for maintaining red cell membrane stability and flexibility, allowing erythrocytes to change shape as they traverse microvasculature without rupturing[1][6]. Mutations in the ANK1 gene are the most common cause of hereditary spherocytosis, a condition characterized by fragile, sphere-shaped red blood cells that are prematurely destroyed in the spleen, resulting in hemolytic anemia[1][3][6]. Beyond erythrocytes, ankyrin-1 is also expressed in muscle and brain, contributing to the structural organization of membranes and involvement in specialized domains, such as anchoring of the sarcoplasmic reticulum in muscle via binding to obscurin[5]. Genome-wide studies have linked altered methylation and expression of ANK1 to neurodegenerative pathology, such as Alzheimer’s disease, implicating the gene in broader cellular processes beyond red cell biology[5]. However, ankyrin-1 is not a classical therapeutic target like a receptor, ion channel, or enzyme, and there are currently no approved drugs that act directly on this protein[1][6][5].
