Overview

The anterior insular cortex (AIC) is a critical anatomical subdivision of the insula, located at the junction of the frontal and temporal lobes. It functions as a primary hub for interoception, integrating physiological signals from the body to generate subjective emotional states and conscious awareness (Craig, 2009, PubMed: 19088924). As a core component of the 'salience network,' the AIC plays a vital role in identifying significant environmental stimuli and switching between default-mode and central-executive brain networks (Menon & Uddin, 2010, PubMed: 20571061). Dysregulation of the AIC is implicated in a wide array of psychiatric disorders, most notably in addiction, where it is thought to mediate the conscious urge to consume drugs (Naqvi & Bechara, 2009, PubMed: 19131659). While the AIC itself is a brain structure rather than a single molecular target, it is highly enriched with specific cell types like von Economo neurons and various neurotransmitter receptors, making it a focal point for studies on anxiety, pain, and social cognition. Therapeutic approaches targeting the AIC range from pharmacological modulation of its internal receptors to focal interventions like Transcranial Magnetic Stimulation (TMS) and Deep Brain Stimulation (DBS).
