Anti-apoptotic action

Overview

Anti-apoptotic action is a physiological process rather than a single molecular target, referring to the biological mechanisms that inhibit programmed cell death (apoptosis) [NIH, National Cancer Institute]. This action is primarily mediated by the B-cell lymphoma 2 (Bcl-2) family of proteins and the Inhibitor of Apoptosis Proteins (IAPs), which maintain cellular survival by neutralizing pro-apoptotic signals and preventing the release of cytochrome c from the mitochondria [PubMed: PMC4860824]. In many malignancies, such as chronic lymphocytic leukemia and multiple myeloma, cells pathologically overexpress anti-apoptotic factors to evade death signals and develop resistance to chemotherapy [Nature Reviews Cancer, 2011]. Therapeutic strategies designed to counteract anti-apoptotic action involve small molecule inhibitors, such as BH3 mimetics like Venetoclax, which restore the cell's natural ability to undergo apoptosis by inhibiting pro-survival proteins [StatPearls: Bcl-2 Inhibitors]. While these drugs are highly effective in specific oncology settings, disrupting anti-apoptotic pathways can lead to severe side effects, most notably tumor lysis syndrome due to rapid cell death and various hematologic toxicities [FDA: Venclexta Prescribing Information].

Mechanism of action

Drugs targeting this process primarily act as BH3-mimetics that competitively bind to the hydrophobic grooves of anti-apoptotic proteins (e.g., Bcl-2, Mcl-1), preventing them from sequestering pro-apoptotic factors and thus inducing mitochondrial outer membrane permeabilization (MOMP) and subsequent cell death.
