Anti-B blood group antibody (Anti-B)

Anti-B blood group antibodies are naturally occurring isoagglutinins found in the serum of individuals with blood group A or O (StatPearls, 2023). These antibodies, predominantly of the IgM and IgG classes, are directed against the B-antigen, a terminal galactose carbohydrate moiety expressed on red blood cells and vascular endothelium (NCBI, 2005). In the context of transfusion medicine and organ transplantation, Anti-B antibodies represent a significant barrier, as they can trigger hyperacute rejection or acute hemolytic transfusion reactions through complement activation and red cell lysis (PubMed, 2021). Therapeutic interventions often target these antibodies to facilitate ABO-incompatible procedures, utilizing methods such as plasmapheresis for physical removal or pharmacological agents like Rituximab to deplete the B-cell populations responsible for their production (Transplantation Reviews, 2016). Monitoring Anti-B titers is a standard clinical practice to assess the risk of immune-mediated complications in sensitized patients (Journal of Clinical Apheresis, 2019).
