Overview

Anti-biotinylated red blood cell (BioRBC) antibodies are immunoglobulins, primarily of the IgG1 subclass, that specifically recognize and bind to biotinylated epitopes on the surface of erythrocytes. These antibodies are of significant clinical interest because biotinylation is a widely used technique for tracking red cell survival (RCS) and volume in hematologic research and clinical trials, serving as a non-radioactive alternative to chromium-51 labeling (Mock et al., 2022; Widness et al., 2017). While approximately 0.5% of the population may possess pre-existing anti-biotin antibodies, exposure to BioRBCs can induce an immune response in up to 15% of subjects, particularly when high biotin label densities are used (Mock et al., 2011). The binding of these antibodies to the biotin-epitope triggers the accelerated clearance of the labeled cells from the bloodstream, typically through extravascular hemolysis or complement activation (Mock et al., 2022). This accelerated removal can lead to a significant underestimation of red cell survival, confounding the results of kinetic studies and posing a challenge for the development of RBC-based drug delivery systems (Widness et al., 2017). Management of this immunological response involves screening patients with sensitive gel card assays and optimizing biotinylation protocols to use the lowest effective label density to minimize immunogenicity (Mock et al., 2011).
