Anti-human platelet antigen 1a (Anti-HPA-1a) antibodies are maternal alloantibodies that specifically target the HPA-1a epitope on the platelet integrin beta-3 subunit (ITGB3) [1, 2]. These antibodies develop when an HPA-1a-negative mother is sensitized to the HPA-1a antigen on fetal platelets, typically during pregnancy or delivery [1]. Once formed, these IgG antibodies can cross the placenta and mediate the destruction of fetal platelets via opsonization and phagocytosis in the fetal spleen [1, 5]. This process leads to Fetal and Neonatal Alloimmune Thrombocytopenia (FNAIT), a condition characterized by severe bleeding risks, most notably intracranial hemorrhage [1]. In drug development, Anti-HPA-1a antibodies are considered a target for clearance or neutralization [3, 4]. Therapeutic approaches include the use of neonatal Fc receptor (FcRn) blockers to accelerate the degradation of maternal IgG and the administration of prophylactic monoclonal antibodies to prevent the initial maternal immune response [3, 4].
Therapeutic strategies involve the reduction of circulating antibody levels through FcRn inhibition, neutralization of the antibody's binding site, or prevention of maternal sensitization through passive immunization.
