Overview

Anti-myelin-associated glycoprotein (MAG) IgM autoantibodies are IgM class autoantibodies that target myelin-associated glycoprotein, a component of the myelin sheath in the peripheral nervous system. These antibodies are pathogenic and associated with autoimmune demyelinating neuropathy. They disrupt axon-Schwann cell interactions and impair myelination, leading to demyelination and axonal loss. They are detected in patients with slowly progressive distal sensory or sensorimotor neuropathies and may be associated with MGUS or Waldenström's macroglobulinemia. Diagnosis involves measuring serum antibody levels, and treatment options are limited with variable efficacy.
