Overview

Antigen-presenting cells (APCs) of the oral mucosa are a specialized group of immune cells, primarily consisting of Langerhans cells and various dendritic cell subsets, that reside within the oral epithelium and lamina propria (NIH). These cells act as critical sentinels that capture exogenous antigens, such as pathogens or allergens, and migrate to regional lymph nodes to initiate or modulate immune responses (ResearchGate). In the oral cavity, these APCs are uniquely adapted to promote immune tolerance, a process essential for preventing hypersensitivity to food and environmental antigens (Abcam). This tolerogenic property is exploited in sublingual immunotherapy (SLIT), where allergens are delivered to the oral mucosa to be processed by APCs, leading to the induction of regulatory T cells (Tregs) and long-term desensitization (NIH). Beyond allergy, oral APCs are involved in the surveillance of oral cancers and the management of mucosal infections (Cancer Research UK). Therapeutic targeting of these cells often involves mucoadhesive formulations or the use of specific ligands to enhance antigen uptake and ensure effective immune modulation (NIH).
