Overview

The Antithrombin III–Factor Xa axis is a fundamental regulatory component of the human coagulation system, where Antithrombin III (ATIII) serves as the primary endogenous inhibitor of the serine protease Factor Xa (FXa) [PMID: 25659655]. ATIII, a member of the serpin superfamily, neutralizes FXa by forming a stable 1:1 stoichiometric complex, a process that is relatively slow under physiological conditions but is accelerated several thousand-fold in the presence of heparin or heparin-like glycosaminoglycans [UniProt: P01008]. Factor Xa occupies a central position in the coagulation cascade, acting as the rate-limiting step in the generation of thrombin from prothrombin [PMID: 11566018]. Pharmacological targeting of this axis, primarily through indirect Factor Xa inhibitors like fondaparinux and low-molecular-weight heparins, leverages the inhibitory capacity of ATIII to provide potent anticoagulation [StatPearls: NBK525961]. This axis is clinically significant in the management of thrombotic disorders such as deep vein thrombosis, pulmonary embolism, and acute coronary syndromes [PMID: 22303008]. Therapeutic monitoring of drugs acting on this axis is typically achieved through the anti-Xa assay, which measures the functional activity of the ATIII-drug complex against FXa [PMID: 30231310].
