Consequently, pharmacological treatments for anxiety target multiple distinct proteins, most notably the GABA-A receptor (via benzodiazepines), the serotonin transporter (via SSRIs), and the 5-HT1A receptor (via buspirone). Identifying a singular "target" for anxiety is clinically inaccurate, as the condition represents a systemic neurobiological imbalance rather than a single disrupted molecule.
Pharmacological interventions for anxiety act through various mechanisms depending on the drug class: 1) Positive allosteric modulation of GABA-A receptors to enhance inhibitory neurotransmission; 2) Selective inhibition of the serotonin transporter (SERT) to increase synaptic serotonin; 3) Dual inhibition of serotonin and norepinephrine transporters (SNRIs); 4) Partial agonism of the 5-HT1A receptor; and 5) Inhibition of the alpha-2-delta subunit of voltage-gated calcium channels to reduce excitatory neurotransmitter release.
