It is not a specific molecular target like a receptor or enzyme but is instead a broad clinical diagnosis encompassing several psychiatric conditions, including generalized anxiety disorder (GAD), panic disorder, and social anxiety disorder. The biological basis of anxiety involves the dysregulation of various neurotransmitter systems and neural circuits, particularly those connecting the amygdala, prefrontal cortex, and hippocampus. Consequently, pharmacological treatments for anxiety target multiple distinct proteins, most notably the GABA-A receptor (via benzodiazepines), the serotonin transporter (via SSRIs), and the 5-HT1A receptor (via buspirone). Identifying a singular "target" for anxiety is clinically inaccurate, as the condition represents a systemic neurobiological imbalance rather than a single disrupted molecule.

Mechanism of action

Pharmacological interventions for anxiety act through various mechanisms depending on the drug class: 1) Positive allosteric modulation of GABA-A receptors to enhance inhibitory neurotransmission; 2) Selective inhibition of the serotonin transporter (SERT) to increase synaptic serotonin; 3) Dual inhibition of serotonin and norepinephrine transporters (SNRIs); 4) Partial agonism of the 5-HT1A receptor; and 5) Inhibition of the alpha-2-delta subunit of voltage-gated calcium channels to reduce excitatory neurotransmitter release.
