Apolipoprotein A-IV (APOA4) is a plasma glycoprotein primarily synthesized in the intestine and secreted on the surface of newly formed chylomicrons, as well as incorporated into high-density lipoproteins (HDL). APOA4 acts as an important mediator of lipid transport and metabolism, facilitating cholesterol efflux via ATP-binding cassette transporter A1 (ABCA1), activating cholesterol esterification through LCAT, and exerting antioxidant effects by protecting low-density lipoprotein (LDL) from oxidation. It also plays roles in the regulation of appetite, satiety, and has anti-atherogenic and anti-inflammatory properties. Genetic polymorphisms in APOA4 may influence lipid metabolism and cardiovascular risk, and raised APOA4 levels have been associated with a reduction in atherosclerosis in animal models.
Drugs or interventions (experimental) may aim to elevate or mimic APOA4 to promote reverse cholesterol transport or inhibit lipid oxidation (mechanism: enhancement of cholesterol efflux, LCAT activation, and antioxidant effects)
