Overview

Apolipoprotein C-IV (APOC4) is a small plasma lipid-binding protein primarily secreted by the liver as part of the apolipoprotein C gene family, with minor expression in the intestine[1][3][5]. APOC4 is present at much lower plasma concentrations than other apolipoprotein Cs (APOC1, APOC2, APOC3), associating mainly with triglyceride-rich lipoproteins (VLDL, chylomicrons) and high-density lipoproteins (HDL). It participates in the modulation of lipid metabolism, likely by affecting plasma triglyceride concentrations and VLDL clearance, but its precise physiological and pathological functions remain poorly defined compared to other APOCs[1][3][5]. Genetic polymorphisms in APOC4 may influence circulating lipid levels and have been associated with a modestly increased risk of coronary artery disease and hyperlipidemia in some populations[1][3]. There are currently no known drug molecules that specifically and directly target APOC4, and it is not a major established therapeutic target[3][5].
