Overview

Apolipoprotein E2 (APOE2) is a variant of the APOE protein (UniProt P02649) that plays a crucial role in lipid metabolism and neuroprotection within the central nervous system (PubMed: 21715603). Unlike the APOE4 isoform, which increases the risk of Alzheimer's disease, APOE2 is associated with a significantly lower risk and a later age of onset (PubMed: 31900391). The interaction between APOE2 and the Amyloid-beta (Aβ) peptide is central to this protective effect, as APOE2 enhances the proteolytic clearance of Aβ and reduces its propensity to aggregate into neurotoxic plaques (PubMed: 21715603, 24140577). Current therapeutic efforts, such as the gene therapy LX1001 developed by Lexeo Therapeutics, focus on introducing the APOE2 gene into the brains of individuals at high risk, such as APOE4 homozygotes, to shift the brain's environment toward an APOE2-dominant state (Lexeo Therapeutics, 2024). By promoting Aβ clearance and potentially stabilizing synapses, this target interaction offers a disease-modifying approach to treating neurodegenerative conditions like Alzheimer's disease and cerebral amyloid angiopathy.
