The apoptosis cascade is a highly regulated sequence of molecular events leading to programmed cell death, essential for maintaining cellular homeostasis and eliminating damaged or redundant cells (StatPearls, 2023). It is primarily divided into the intrinsic (mitochondrial) pathway, regulated by the BCL-2 family of proteins, and the extrinsic (death receptor) pathway, triggered by external ligands like TNF-alpha (NIH, 2022). Both pathways converge on the activation of executioner caspases, such as Caspase-3, which orchestrate the systematic dismantling of the cell (PubMed, 2021). In many diseases, particularly cancer, this cascade is suppressed, allowing for uncontrolled cell proliferation and survival (Nature Reviews Cancer, 2020). Conversely, excessive activation of the apoptosis cascade is a hallmark of neurodegenerative disorders and ischemic injuries. Therapeutic strategies often focus on specific components within the cascade, such as BCL-2 inhibitors like venetoclax, to restore apoptotic sensitivity in malignant cells (FDA, 2016). Understanding the balance between pro- and anti-apoptotic signals is crucial for developing effective treatments that minimize off-target effects on healthy tissues.
Modulation of the apoptosis cascade involves the pharmacological activation or inhibition of specific pathway components, such as the inhibition of anti-apoptotic BCL-2 proteins to trigger mitochondrial outer membrane permeabilization (MOMP) and subsequent caspase activation in cancer cells (Nature Reviews Drug Discovery, 2018).
