Overview

Apoptosis-regulating kinases (also referred to as apoptosis/anti-apoptosis kinases) are a diverse group of enzymes that act as key molecular switches in the signaling pathways governing cell survival and programmed cell death. These kinases are fundamentally divided into two functional categories: anti-apoptotic kinases, such as Akt (Protein Kinase B), ERK1/2, and PIM kinases, which promote survival; and pro-apoptotic kinases, such as Apoptosis signal-regulating kinase 1 (ASK1), c-Jun N-terminal kinases (JNK), and Death-associated protein kinases (DAPK), which trigger the apoptotic cascade (PMC: 5814546, PubMed: 30043831). In many diseases, this balance is disrupted; for instance, many cancers are characterized by the overactivation of anti-apoptotic kinases, allowing tumor cells to evade death and resist chemotherapy (PubMed: 12591873). Conversely, excessive activity of pro-apoptotic kinases is implicated in the neuronal loss seen in neurodegenerative disorders and tissue damage in cardiovascular disease. Small-molecule inhibitors like capivasertib and selonsertib represent therapeutic attempts to modulate these pathways. Clinical challenges include systemic toxicities such as hyperglycemia and potential cardiotoxicity due to the widespread role of these kinases in normal metabolic and survival signaling (PMC: 4332306).

Mechanism of action

Drugs targeting this class typically function as small-molecule inhibitors that competitively bind to the ATP-binding site or allosteric sites of the kinase, thereby preventing the phosphorylation of downstream effector proteins. Inhibition of anti-apoptotic kinases (e.g., Akt, PI3K) serves to remove survival signals and sensitize diseased cells to apoptosis, whereas the inhibition of pro-apoptotic kinases (e.g., ASK1, JNK) aims to prevent inappropriate cell death in degenerative conditions.
