Overview

Autologous tumor stem cell antigens represent a patient-specific pool of proteins and peptides derived from self-renewing cancer stem cells (CSCs) or tumor-initiating cells (TICs) [5, 14]. These antigens are harvested from a patient's own resected tumor, which is cultured ex vivo to enrich for the stem cell population before being processed into a lysate or used to pulse dendritic cells for personalized vaccines [4, 14]. By targeting the CSC compartment, these therapies aim to eradicate the specific subpopulation of cells responsible for tumor recurrence, metastasis, and resistance to standard treatments like chemotherapy and radiation [1, 12, 13]. This approach utilizes the uniqueness of personal neoantigens and shared CSC-associated markers to induce a potent, targeted T-cell mediated immune response [2, 10]. Clinically, these antigens serve as the therapeutic target for investigational immunotherapies such as AV-GBM-1 in glioblastoma and other personalized vaccines for melanoma and ovarian cancer [7, 9]. The autologous nature of the antigens helps to address intra-tumor heterogeneity while potentially reducing the risk of off-target toxicity [4, 6, 15].

Mechanism of action

Stimulation of a patient-specific immune response via the ex vivo loading of dendritic cells with autologous cancer stem cell antigens, which then present these epitopes to T-cells to induce a targeted cytotoxic attack against the self-renewing tumor cell population [1, 4, 11].
