B-cell lineage surface antigens are a diverse group of proteins expressed on the surface of B lymphocytes at various stages of development, including CD19, CD20, CD22, and CD79b (UniProt). These antigens play essential roles in B-cell receptor signaling, activation, and survival (NCBI). In the context of Epstein-Barr virus (EBV) infection, the virus primarily targets and persists within B cells, which can lead to the development of EBV-associated lymphoproliferative disorders (EBV-LPD) or lymphomas such as Burkitt lymphoma (NIH). Therapeutic strategies targeting these surface antigens are widely used to treat these conditions; for instance, the anti-CD20 monoclonal antibody rituximab is a standard treatment for EBV-LPD (StatPearls). Additionally, chimeric antigen receptor (CAR) T-cell therapies targeting CD19 or CD22 are employed to eliminate malignant B-cell populations (PubMed). While B-cell lineage antigens are host proteins, they are distinct from EBV-specific antigens (such as LMP1 or LMP2) which are also targeted by specialized T-cell therapies like tabelecleucel in EBV-positive diseases (EMA). Targeting these antigens allows for the selective depletion of both normal and malignant B cells, though this often results in prolonged B-cell aplasia and secondary immunodeficiency (NIH).
