Overview

The B-cell lymphoma patient-specific idiotype is the unique set of antigenic determinants (idiotopes) present in the variable region of the immunoglobulin expressed as the B-cell receptor (BCR) on the surface of malignant B cells in each individual with B-cell lymphoma[1][3]. These determinants arise from somatic hypermutation and V(D)J gene rearrangement, making the idiotype a clonal marker and, in principle, a tumor-specific antigen. Therapeutic strategies involve generating idiotype vaccines or monoclonal antibodies that target this antigen, aiming to elicit a highly specific anti-tumor immune response. However, intra-tumor heterogeneity and somatic variation often complicate treatment efficacy as not all malignant cells may express the same idiotype[2]. This approach remains a model for personalized cancer immunotherapy, in which each patient’s lymphoma idiotype serves as both a diagnostic biomarker and a therapeutic target[1][2][3].
