Bacterial and follicular cellular components in Cutibacterium acnes and skin refers to the complex pathological environment of the pilosebaceous unit involved in the development of acne vulgaris. Cutibacterium acnes (formerly Propionibacterium acnes) is a Gram-positive anaerobic bacterium that colonizes the hair follicle, where it interacts with host sebocytes and keratinocytes (Fitz-Gibbon et al., 2013). The bacterium contributes to inflammation by activating the innate immune system, specifically through Toll-like receptor 2 (TLR2) on macrophages and keratinocytes, which triggers the release of pro-inflammatory cytokines like IL-12 and IL-8 (Kim et al., 2002). Follicular components contribute to the disease state through hyperkeratinization and excessive sebum production, creating an anaerobic niche that promotes bacterial proliferation (StatPearls, 2023). Pharmacological interventions target these components collectively using antibiotics to reduce bacterial load, retinoids to normalize cell turnover, and benzoyl peroxide to induce oxidative stress in the bacteria (Zaenglein et al., 2016). Because this entry describes a multi-factorial biological system rather than a single molecular entity or receptor, it is classified as a system-level description rather than a discrete therapeutic target.
