Overview

This target category comprises essential structural components of microbial cells, specifically the negatively charged phospholipids and phosphate-containing polymers found in bacterial and fungal membranes and cell walls. In bacteria, key components include phosphatidylglycerol, cardiolipin, and teichoic acids in Gram-positive organisms, as well as the lipid A portion of lipopolysaccharides in Gram-negative organisms (Swoboda et al., 2010; Zavascki et al., 2007). These molecules are vital for maintaining the electrochemical gradient, osmotic stability, and structural integrity of the cell (Heidary et al., 2022). In fungi, phosphorylated mannans and membrane lipids serve similar roles in cell wall architecture and signaling (McKenzie et al., 2010). Therapeutic agents like polymyxins and daptomycin target these components through electrostatic interactions with the anionic phosphate groups, leading to membrane pore formation, depolarization, and rapid cell death (Humphries et al., 2013). Because these targets are largely absent or differently organized in mammalian cells, they provide a basis for selective toxicity. However, high doses or systemic accumulation can lead to adverse effects such as nephrotoxicity and neurotoxicity due to interactions with host cell membranes (StatPearls, 2023). Monitoring of biomarkers like procalcitonin and specific microbial antigens is often used to guide therapy targeting these structures.
