Overview

Lipid A is the highly conserved, hydrophobic anchor of lipopolysaccharide (LPS) situated within the outer leaflet of the outer membrane of Gram-negative bacteria. It is widely recognized as the 'endotoxin' responsible for the potent inflammatory response associated with Gram-negative bacterial infections. Biologically, Lipid A is essential for bacterial viability and membrane stability, but it also serves as a primary pathogen-associated molecular pattern (PAMP) that triggers the human innate immune system through the TLR4/MD-2 receptor complex. In clinical medicine, Lipid A is a critical therapeutic target for polymyxin antibiotics, which bind directly to the molecule to disrupt bacterial membrane integrity. However, the overstimulation of the immune system by Lipid A can lead to lethal sepsis and septic shock, making its neutralization or the inhibition of its biosynthetic pathway (such as the LpxC enzyme) a major focus for drug development against multi-drug resistant pathogens.

Mechanism of action

Drugs like polymyxins bind to the negatively charged phosphate groups of Lipid A via electrostatic interactions, displacing divalent cations (Mg2+ and Ca2+) that stabilize the outer membrane. This displacement leads to membrane disorganization, increased permeability, and eventual cell lysis. Other agents, such as Eritoran, act as competitive antagonists at the TLR4/MD-2 complex, mimicking the structure of Lipid A to prevent endotoxin-induced signaling.
