Overview

Bacterial superantigens (SAgs) are a family of potent exotoxins secreted primarily by Gram-positive bacteria, most notably Staphylococcus aureus and Streptococcus pyogenes (Source: StatPearls, PMID: 31082117). Unlike conventional antigens that are processed and presented within the peptide-binding groove of Major Histocompatibility Complex (MHC) molecules, SAgs bind directly to the invariant regions of MHC class II and the Vβ chain of the T-cell receptor (TCR) (Source: Nature Reviews Microbiology, PMID: 11905331). This unconventional binding bypasses normal antigen specificity, triggering the massive, non-specific activation of up to 20-30% of the total T-cell population (Source: Frontiers in Immunology, PMID: 28261217). The resulting systemic release of pro-inflammatory cytokines, such as TNF-alpha, IL-1, and IFN-gamma, leads to a cytokine storm that can cause toxic shock syndrome, hypotension, and multi-organ failure (Source: Clinical Microbiology Reviews, PMID: 11158129). Therapeutic strategies focus on neutralizing the toxins using intravenous immunoglobulins (IVIG) or utilizing antibiotics like clindamycin that suppress toxin synthesis (Source: Journal of Infectious Diseases, PMID: 15170531).
