Overview

Bacterial toxins and other unwanted luminal compounds represent a broad category of pathogenic substances located within the lumen of the gastrointestinal tract or other body cavities. This group includes bacterial exotoxins, such as those produced by Clostridioides difficile or Vibrio cholerae, as well as endotoxins like lipopolysaccharides (LPS) and metabolic waste products like uremic toxins (Nature Reviews Nephrology, 2021). These substances contribute to disease by damaging the mucosal lining, triggering systemic inflammation, or causing secretory diarrhea (NEJM, 2015). Therapeutic strategies involve the use of non-absorbable binders and adsorbents, such as activated charcoal or ion-exchange resins, to sequester these molecules and prevent their absorption or local action (StatPearls, 2023). Additionally, specific monoclonal antibodies, such as bezlotoxumab, are used to neutralize high-potency bacterial toxins directly (FDA, 2016). While not a single molecular target, managing the luminal environment is a critical therapeutic approach in infectious diseases, nephrology, and gastroenterology.
