Overview

Beta-1 adrenergic receptor autoantibodies (β1-AABs) are pathogenic immunoglobulins that target the beta-1 adrenergic receptor (ADRB1), primarily in the heart (Wallukat & Schimke, 2014, PubMed: 24633490). These autoantibodies often act as functional agonists, binding to the second extracellular loop of the receptor and inducing chronic, unregulated signaling (Jahns et al., 2004, PubMed: 14732753). This persistent activation leads to cardiomyocyte damage, calcium mishandling, and cardiac remodeling, which are hallmarks of dilated cardiomyopathy and heart failure (Müller et al., 2000, PubMed: 10653414). β1-AABs are found in a significant percentage of patients with idiopathic dilated cardiomyopathy and are considered a key driver of disease progression (Störk et al., 2006, PubMed: 16412446). Therapeutic interventions include immunoadsorption to physically remove the antibodies from circulation and the development of neutralizing agents like the DNA aptamer BC007 (Berlin Cures, 2023). By neutralizing these autoantibodies, clinicians aim to restore normal adrenergic sensitivity and improve hemodynamic function in heart failure patients.
