Beta-lactam antibiotics in the gastrointestinal lumen refer to the residual concentrations of systemically administered antibiotics (such as penicillins and cephalosporins) that are excreted via the bile or incomplete absorption into the intestinal tract. While these drugs are intended to treat systemic infections, their presence in the gut lumen inadvertently kills beneficial commensal bacteria, leading to dysbiosis and increasing the risk of opportunistic infections like Clostridioides difficile. This environment also promotes the selection and spread of antimicrobial resistance genes among the gut flora. Therapeutic strategies, such as the use of oral beta-lactamase enzymes like ribaxamase, specifically target and degrade these residual antibiotics within the GI tract to protect the microbiome without affecting the systemic therapeutic levels of the antibiotic. By neutralizing these molecules before they reach the colon, clinicians aim to prevent antibiotic-associated diarrhea and maintain the integrity of the intestinal ecosystem.
