Bicarbonate generation via hepatic metabolism of L-lactate refers to the process by which the liver metabolizes L-lactate, resulting in the net production of bicarbonate (HCO₃⁻). This is a crucial physiological mechanism for maintaining acid-base balance, especially during conditions where lactate levels are elevated, such as exercise or critical illness. It involves the conversion of L-lactate to pyruvate by lactate dehydrogenase, followed by gluconeogenesis or oxidation through the TCA cycle. Both pathways consume protons and result in net bicarbonate generation. Impaired hepatic function reduces the capacity for this process, predisposing patients to lactic acidosis.
