Overview

The biliary tract, also known as the biliary tree, comprises the ducts that convey bile from the liver and gallbladder to the duodenum. In medical imaging, "contrast enhancement" of the biliary tract refers to techniques that use contrast agents to improve visualization and assessment of these ducts during radiological examinations. Contrast enhancement is not a molecular target or receptor in itself but rather a process or imaging phenomenon. It involves administering contrast agents—substances that alter how tissues appear on imaging modalities such as ultrasound (US), computed tomography (CT), or magnetic resonance imaging (MRI)—to highlight anatomical structures and pathological changes within the biliary system.

Mechanism of action

Contrast enhancement relies on the pharmacokinetics of specific agents—how they are absorbed by hepatocytes and secreted into bile—and their ability to increase signal intensity within normal/abnormal segments of the biliary tree on various imaging sequences. For US/CEUS: Microbubbles remain intravascular; their echogenicity highlights vascular patterns associated with lesions. For MRI/CT: Agents may be selectively taken up/excreted by hepatocytes versus renal clearance; this differential allows targeted visualization.
