Biomarkers of bone formation are biochemical molecules released during the activity of osteoblasts (bone-forming cells). The most clinically relevant markers include **P1NP** (procollagen type I N-terminal propeptide), **bone-specific alkaline phosphatase**, and **osteocalcin**. These proteins serve as indirect measures of new bone matrix synthesis and mineralization. They are routinely used, particularly in osteoporosis, to monitor bone formation response to therapy and disease progression. However, "biomarkers of bone formation" is not itself a druggable target or gene, but rather refers to a set of analytes measured in body fluids to reflect anabolic bone activity. Their utility is principally as surrogate markers for patient selection or efficacy monitoring, not as interventional targets themselves. Their levels can be modified by various physiological and pathological states, and are recommended for therapy monitoring rather than as primary diagnostic or prognostic tools.
