Overview

The bladder urothelial glycosaminoglycan (GAG) layer is a specialized, highly hydrophilic coating of sulfonated polysaccharides—including hyaluronic acid, chondroitin sulfate, and heparan sulfate—that lines the luminal surface of the bladder urothelium (Parsons, 2007). Its primary biological function is to serve as a non-adherent barrier that prevents bacteria and irritating urinary solutes, such as potassium ions and metabolic waste, from penetrating the bladder wall (Hurst et al., 2015). In pathological states like interstitial cystitis/bladder pain syndrome (IC/BPS), the GAG layer is often deficient or damaged, leading to increased urothelial permeability and subsequent chronic inflammation and pain (StatPearls, 2023). Therapeutic interventions aim to restore this barrier through GAG substitution therapy, utilizing oral agents like pentosan polysulfate sodium or intravesical instillations of heparin and hyaluronic acid (FDA, 2020). While effective for many patients, long-term use of certain systemic GAG-targeting drugs has been linked to specific safety concerns, such as pigmentary maculopathy.
