Overview

The bladder urothelial glycosaminoglycan (GAG) layer is a protective coating of mucopolysaccharides, including heparin sulfate and hyaluronic acid, located on the luminal surface of the bladder (Parsons, 2007, PMID: 17499305). Its primary biological function is to act as a highly hydrophilic barrier that prevents the adherence of bacteria and the penetration of toxic urinary solutes, such as potassium ions, into the underlying tissue (Hurst, 1994, PMID: 8158771). In patients with interstitial cystitis/bladder pain syndrome (IC/BPS), this GAG layer is often deficient or dysfunctional, leading to increased urothelial permeability and subsequent chronic inflammation and pain (StatPearls, 2023, NBK470312). Therapeutic strategies involve GAG replenishment therapy, where exogenous GAG analogs like pentosan polysulfate sodium or intravesical hyaluronic acid are used to restore the barrier integrity and alleviate symptoms (FDA Elmiron Label; Madersbacher et al., 2013, PMID: 23107468). This target is critical for managing chronic bladder inflammatory conditions by addressing the underlying structural defect in the urothelial defense mechanism.
