Overview

The bladder urothelial surface glycosaminoglycan (GAG) layer is a thin, hydrophilic coating of polysaccharides—primarily hyaluronic acid, chondroitin sulfate, and heparan sulfate—that lines the luminal surface of the bladder (Parsons, 2007). Its fundamental biological role is to act as a non-specific anti-adherent barrier, preventing bacteria, microcrystals, and toxic urinary solutes like potassium from penetrating the urothelium (Hurst, 1994). In patients with interstitial cystitis/bladder pain syndrome (IC/BPS), this layer is frequently deficient or damaged, leading to increased permeability and the subsequent activation of suburothelial sensory nerves and inflammatory pathways (Hanno et al., 2015). Pharmacological management often targets this layer through GAG replenishment therapy, which aims to restore the barrier integrity. Common therapeutic agents include oral pentosan polysulfate sodium, which is thought to adhere to the bladder wall, and intravesical instillations of heparin, hyaluronic acid, or chondroitin sulfate (FDA, 2020).
