Overview

Blood-brain barrier (BBB) endothelial cell receptors are a diverse group of proteins expressed on the luminal surface of brain capillary endothelial cells that facilitate the transport of essential molecules into the central nervous system (CNS). These receptors, including the transferrin receptor (TfR), insulin receptor (IR), and low-density lipoprotein receptor-related protein 1 (LRP1), primarily function through receptor-mediated transcytosis (RMT), a process where ligands are internalized and transported across the cell to the brain parenchyma. In the context of drug development, these receptors are critical targets for Trojan horse strategies, where therapeutic agents are conjugated to ligands or antibodies that bind these receptors to bypass the otherwise impermeable BBB. Dysfunction or altered expression of these receptors is associated with various neurological conditions, such as Alzheimer's disease and stroke, where the integrity of the BBB is compromised. Targeting these receptors offers a non-invasive route for delivering biologics, such as monoclonal antibodies and enzymes, to treat CNS disorders, though challenges remain regarding off-target binding and receptor saturation.
