Overview

Blood-brain barrier (BBB) endothelial receptors are a specialized group of membrane proteins expressed on the luminal surface of brain capillary endothelial cells. These receptors, which include the transferrin receptor (TfR), insulin receptor (IR), and low-density lipoprotein receptor-related protein 1 (LRP1), facilitate the transport of essential large molecules from the blood into the brain parenchyma via receptor-mediated transcytosis (RMT). In pharmacology, these receptors are exploited as "Trojan horse" targets to deliver therapeutic agents—such as monoclonal antibodies, enzymes, or oligonucleotides—that otherwise cannot cross the highly restrictive BBB. By binding to these receptors, drug conjugates are internalized into endocytic vesicles, transported across the endothelial cell, and released into the brain. This approach is critical for treating neurodegenerative diseases, brain tumors, and metabolic disorders affecting the central nervous system. However, targeting these receptors requires careful design to avoid interfering with the transport of endogenous ligands like iron or insulin, which could lead to systemic or localized toxicity.
