Overview

Blood-level pathogenic heat, or Xue Fen Re, is a pathological syndrome in Traditional Chinese Medicine (TCM) representing the terminal and most critical stage of the Wen Bing (Warm Disease) framework (Ye Tianshi, 1746). It is characterized by the invasion of pathogenic heat into the blood, resulting in extreme hyperthermia, damage to the vascular system, and disturbance of the spirit (Shen). Clinically, this manifests as a deep red tongue, maculopapular rashes, and various hemorrhagic symptoms such as epistaxis or hematemesis. In modern biomedical terms, this state is often associated with systemic inflammatory response syndrome (SIRS), sepsis, and disseminated intravascular coagulation (DIC) (PMID: 32431770). Therapeutic intervention focuses on clearing heat and cooling the blood using herbal agents like Radix Rehmanniae and Cortex Moutan, which have been shown to modulate pro-inflammatory cytokines and protect endothelial integrity (PMID: 28655525). Because it is a complex syndrome rather than a single protein or receptor, it does not fit the standard definition of a molecular drug target.

Mechanism of action

The therapeutic approach involves clearing heat and cooling the blood (Qing Re Liang Xue). Modern pharmacological studies suggest that the constituent compounds in associated TCM formulas, such as catalpol and paeonol, act by inhibiting pro-inflammatory cytokine storms (e.g., TNF-alpha, IL-6) and preventing the activation of the extrinsic coagulation pathway (PMID: 28655525).
